This notice is to inform you that your
IRS Form 1095-C for the 2025 tax year is now available. Form 1095-C provides information about the health insurance coverage offered to you by BOJ of WNC.

This form may be helpful when completing your federal tax return, but you are not required to submit it with your return.


How to request your form:

Request an Electronic Copy:  Send an email to hr@bocountry.com with the subject line of Form 1095C Request. Provide your name and email address where we can email your form. 

Request a Paper Copy:  Send an email to hr@bocountry.com with the subject line of Form 1095CRequest. Provide your name and home address where we can mail your form. 


Why You Receive This Form

Under the Affordable Care Act (ACA), large applicable employers are required to provide employees with Form 1095-C to document:

  • The health coverage offered to you
  • Your enrollment (if applicable)
  • The months you were eligible for coverage
